FINANCIAL AGREEMENTS

MONTHLY TUITION

I understand and agree that all tuition payments are due on or before the 15th of each month. (January - December). Payments are to be made exclusively via invoice as provided by LDW Studios.

I acknowledge that a $25 late fee will automatically be charged to my account for any payments received after the due date.

I fully understand that neither I nor my dancer will be permitted to participate in any LDW Studio classes, events, or performances until all outstanding balances, inlcuing tuition, late fees, are paid in full.

By signing below, I agree to abide by this financial policy.

FUNDRAISING AGREEMENT

As part of the LDW team, I understand that fundraising is an essential component to help support team activities, events, uniforms, costumes, and performances.

I understand there are no refunds for any funds raised.

TEAM FUNDRAISING EFFORTS

• Funds raised for a specific purpose (such as costumes, travel expenses, supplies, etc) will be used solely for that purpose. No funds will be transferred for any other purposes, unless approved by the financial administrator and director.

I agree to the following:

1. Participation:

   All dancers are required to participate in all team/program fundraising efforts.

2. Fundraising Minimum:

   Each dancer must meet the minimum fundraising goals set forth per fundraising campaign.

3. Buy-Out Option:

   Families may choose to opt out of fundraising efforts by paying a buy-out fee set forth per fundraising campaign.

4. Combination Option:

   If a dancer partipates in fundraising but does not meet the minimum goal, I understand that the difference between the amount raised and the minimum goal amount will be my financial responsibility, and I agree to pay the remaining balance.

   
   

INDIVIDUAL FUNDRAISING ACKNOWLEDGEMENT

I acknowledge that individual fundraising efforts are designed to directly benefit my dancer's participation in the LDW dance program.

• A percentage of funds raised will be credited to my dancer's personal financial account. These funds can be applied toward my dancer's uniforms, costumes, travel expenses, monthly tuition and any outstanding balances related to their participation in the program.

• I understand that funds raised are non-transferable and are strictly for my dancer's program-related expenses.

Additionally:

• The financial administrator will automatically apply the raised funds to any outstanding or upcoming expenses as necessary to ensure all dues are paid in full by the required due dates.

By signing below, I agree to the terms outlined above and understand the purpose and application of both team and individual fundraising efforts.

UNIFORM AND TEAM ATTIRE

As a member of LDW Studios Performance & Company Teams, I understand and agree to the following policies regarding uniforms, costumes, and all attire.

1. OWNERSHIP:

   All uniforms and costumes are the sole property of LDW Studios. Dancers will have access to the necessary attire for all performances.

2. CARE AND MAINTENANCE:

   Dancers are responsible for the care, maintenance, and cleaning of all uniforms and costumes while in their possession. LDW is not responsible for any attire that is lost, stolen, or damaged while in the care of the dancer.

3. REPLACEMENT RESPONSIBILITY:

   In the event that a uniform, costume, or any LDW attire is lost, stolen, or damaged, the dancer will be responsible for covering the full cost of replacement.

4. STORAGE AND DISTRIBUTION:

   All costumes and uniforms will be stored at the studio and distributed to dancers as deemed necessary by LDW staff. Dancers are required to pick up and return all attire on schedule as set by the studio staff.

   
   

ATTIRE & UNIFORM FEE

For the 2025 dance season, the Attire & Uniform Fee is set at $350. (This does not pertain to our Twinkling Dancers) We will be stepping into season 10 with a new look. The dancers will have several different looks for the season!

This fee includes:

Uniform and costume rental fees for the year

Dance tights

Socks

Hair accessories

Gloves

Practice T-shirt/Attiee

PAYMENT OPTIONS AND FUNDRAISING

• Individual fundraising efforts can assist in covering the out-or-pocket cost for parents and adult dancers.

• Fundraising credits will be applied directly to your dancer's financial account to reduce total fee.

DEADLINES AND ADDITIONAL FEES

• All deadlines for payments and attire orders must be met to ensure timely delivery.

• Failure to meet deadlines may result in additional shipping, handling, or other associated fees, which will be the responsibility of the parent/guardian/dancer.

By signing below, I agree to pay the $350 Attire & Uniform Fee, meet all deadlines, and understand that any additional fees incurred due to missed deadlines will be added to my account.

WAIVER & RELEASE

ACKNOWLEDGMENT OF RISK

I understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities at LDW Studios & The ARTSS involves inherent risks, including but not limited to physical injury, accidents, or illness. I voluntarily assume all risks associated with participation.

RELEASE AND LIABILITY

In consideration of being allowed to participate, I, on behalf of myself and/or my minor chid, release and hold harmless LDW Studios, its owners, instructors, coaches, staff, and volunteers from any and all liability, claims, demands, or causes of action arising from participation, including injury, property damage, or loss.

MEDICAL TREATMENT AUTHORIZATION

In the event of a medical emergency, I authorize LDW Studios to seek appropriate medical care for me or my child. I understand that I am responsible for any medical expenses incurred.

PHOTO/VIDEO RELEASE

I grant permission for LDW Studios to use photos or videos of me/my child for promotional purposes, including social media, website, content, and advertising.

I acknowledge that I have read and agree to abide by all studio policies, above including attire, financial, and all policies set forth in the successful operations of LDW Studios.
